Incident Report

Where an incident has occurred during a club activity which involves abusive behaviour, injury, illness or property damage sustained by any party, or where there was potential for injury, illness or damage, an Incident Report should be completed and submitted as soon as possible after the incident. Please use the form below.

Those involved in the incident should delegate one person to complete the form. One form should be completed per incident.

When filling out the form, please include the following:

  • First name
  • Last name
  • Nature of incident (Property Damage / Verbal or Physical Abuse / Injury / Illness)
  • Location, Date and time of incident
  • Name of Ride / event and Ride Leader / Event Organiser
  • Description of incident
  • Names of those involved
  • Property Damage
  • First aid
  • Club witnesses
  • Independent witnesses
  • Police involvement
  • Additional Information
